We now offer the convenience of making appointments online. If any
of the available times is not available or the nature of your visit is
more urgent, please call our office to speak directly to our staff at
(480) 812-2010 We will make our best
effort to verify your insurance to determine eligibility before you come
in, but we do not guarantee coverage. There may be situations in
which the insurance company cannot be reached (website down, closed for
weekend/holiday) and you may be requested to reschedule your
appointment.
Our commitment to you is to provide the very best exam in a
professional, courteous atmosphere. We do not double/triple book
patients and cause delays on our punctuality. As a return, we
request that you call us if you are not going to be able to make your
appointment due to illness/emergencies.
Please allow for 24 hour notice if you are not able to make your
appointment and wish to cancel/reschedule.
Dr. Loc H. Pham is available on Tuesday (10-6 PM), Thursday (10-6 PM) and
Sunday (12-4 PM).
Dr. Chantelle M. Clarizio is available on Friday and Saturday (10-4 PM).
